    Hilgrad has just submitted some Achievement Flag Info for ELDEN RING Achievements:

    This boss should have the 'Missable' tag - the NPC who provides the questline to reach this boss can be killed (and choosing to kill the NPC is additionally one of the dialogue options that can be selected prior to reaching this boss). The NPC is not able to be revived or reset through the Absolution mechanic - so if the NPC is killed during their questline, this boss is permanently missable until fully completing and restarting that full playthrough.

    The weapon 'Bolt of Gransax' is missable, as the location to get it will be buried under ash once you defeat 'Maliketh, The Black Blade' and will no longer be accessible. Making this achievement missable, since that weapon is required for it.
