While in any campaign hospital after the first level.. On the objectives screen in the top right, move over to the middle section for online. Then click a challenge. I chose cures and it will then automatically select friends from your Steam account to race against to get the most cures. Your friends don't need to play or even own the game. Just wait the days for the challenge to complete.
