    Be careful to click on "HARDCORE" in the option on menu; BEFORE start a new game.

    if you click on "hardcore" even 2s after launching your new game, it will not be taken into account for the trophy.
    You can check that the game is in hardcore if the logo is on your character at the bottom left of the screen
