You need to pick up a piece of armor or a weapon that already has 4 Runes slotted.
I think it popped on a piece of gear I hadn't enhanced yet but I can't confirm.
It's RNG but if you loot everything you should find it first playthrough, I just passively got this on my first playthrough on Normal.
